The Health Coach works as a regular team member, helping clients to gain knowledge, skills, tools and the confidence they need, to take an active part in their own health care.
The Doctors Medplus - You will be expected to work at both our sites in Hauraki and Devonport
This is a permanent position working 32 hours across 4 days per week.
There are five main areas of focus for the Health Coach:
- To provide support for their client to manage their condition themselves;
- To bridge the gap between their client and their GP;
- To help their client find their way around the healthcare system;
- To offer their client emotional support;
- To be their clients on-going first person to contact if they have a question around their health care.
The Health Coach is actively engaged in utilising an integrated 'Stepped Model of Care' that supports clients needs and enables them to move seamlessly between services in general practice, local community based support and services, and if required, DHB and non-government mental health and alcohol services and/or drug services and supports.
This role is complex as it needs to balance the needs of the client with the support structures in place in the Health Sector and local communities, to provide evidence-based interventions to help develop on-going long-term self-sufficiency in clients. Each client will have unique and potentially complex needs and the Health Coach needs to identify appropriate solutions alongside the client and work with the client to enable them to achieve long-term outcomes.
Ideal Candidate:
- Recent experience in primary care, either as a charge/lead nurse or similar
- Great interpersonal skills,
- Accurate documentation and commitment
- Ability to work well as part of a team,
- Able to provide support & encouragement
- Be effective with change management
